Tian Wang Bu Xin Dan Modified (LPI) is suited for Lingering Pathogenic Illness presentations with pronounced exhaustion of Heart and Kidney Yin, leading to disturbed Shen (spirit) and deep fatigue. It is indicated when patients experience persistent insomnia or restless sleep, difficulty falling asleep despite exhaustion, vivid or disturbing dreams, and a sense of agitation or anxiety that worsens in the evening. Cognitive symptoms such as poor memory, difficulty concentrating, and mental cloudiness may be accompanied by palpitations, dry mouth or throat (especially at night), night sweats, heat sensations in the palms and soles, and a red tongue with scanty coat. Joint or body pain may appear in more depleted patients. This modification nourishes Yin and Blood, anchors the Shen, gently clears residual heat, and supports restorative sleep and mental clarity while replenishing the deep reserves depleted by chronic illness.
